Summer Sports Day – Player Form

I acknowledge and agree that I understand the nature of Coach Core Summer Sports Day 2026, hereby referred to as Summer Sports Day; that I am in good health, and in proper physical condition to participate in Summer Sports Day. I agree that participation by me in Summer Sports Day is solely and entirely at my own risk and (to the extent permitted by law) no person(s) or organisation(s) shall be liable for any accident, harm, injury, loss or damage to property, however so caused, arising out of or in connection with, or suffered during as a result of, my participation in Summer Sports Day. During the course of Summer Sports Day, I acknowledge that I may be filmed or photographed. I consent to photographs, filming and sound recording of myself and grant permission, free of charge, to Coach Core Foundation, its licensees and those authorised by it, to use such photographs, films and recordings (including any copies) and for my image, likeness and voice to be included in pictures and/or on film and/or audio broadcasts or transmissions and their exploitation and advertising without compensation or credit. I understand the importance, for safety reasons, of obeying the rules, guidelines and instruction given by Coach Core Foundation and other authorised personnel and hereby agree to abide by instructions given by Coach Core Foundation, and its affiliates which include apprentices and Rosslyn Park employees and staff. I consent to medical assistance and/or medical care being given to me in the case of illness, injury or an emergency situation. I consent to, should this occur during Summer Sports Day, such assistance be given by the organiser, its employees, affiliates or other trained personnel. For these purposes, an organiser is any one or more of the following, Coach Core Foundation, Coach Core Foundation's affiliated parties, Rosslyn Park, their subsidiaries and respective representatives.

I agree to act respectfully and responsibly throughout Summer Sports Day. I will follow event rules and instructions from organisers, treat others fairly and courteously, and help maintain a safe and inclusive environment for everyone. I understand that Coach Core Foundation and Rosslyn Park reserve the right to remove any participant whose behaviour is deemed unsafe, disruptive or inappropriate.
